Family Health Services is a Federally Qualified Health Center (FQHC) that provides quality, affordable primary care, dental, and behavioral health services. Our "high touch, high tech" clinics offer unique patient focused care to anyone in the community, regardless of their ability to pay. We are rapidly growing in Sandusky, Ohio at our two locations as well as in Norwalk, Ohio and surrounding areas. We also offer behavioral health and primary care at several local schools in hopes of eliminating barriers to those needing services.We are looking for a licensed practice nurse (LPN) for our developing Women's Health Program, who would be responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse.Essential Functions

  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse.
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.
  • Maintain and coordinate communication among patient/significant other, staff and physician as evidenced within medical record, client, and health care team feedback.
  • Assist in assuring that effective communication exists with patient/significant other as reflected in the plan of care. Give punctual change of shift report consisting of pertinent and relevant information for assigned patients.
  • Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback.
  • Use available written and AV materials specific to patient condition or disease. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration, or discussion.
  • Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated.

    This role requires regular walking to various locations around the building. This role also routinely comes into contact with patients who may have contagious illnesses.Expected Work Hours

    Monday through Friday 8am-5pm.Travel

    Minimal, with possible occasional travel to other site locations within local area.Required Education and Experience
    • Graduate of an accredited school of Nursing.
    • Valid registration as an LPN in the state.
    • Completion of relevant state Board of Nursing approved.
