Position Requirements:B.A., degree from an accredited college or university or a religious professional (clergy, religious or theologically trained lay person) who has a combination of documented formal training in pastoral counseling and supervised counseling experience in a health care setting the total of academic and supervised work experience must equal at least five (5) years. Excellent communication and human relations skills Ability to work well with individuals of diverse cultural, ethnic, religious and economic backgrounds Knowledge and experience in ministering to persons who are dying and their grieving family and friends. Knowledge of adult learning principles. Two (2) units of CPE, accredited by ACPE NACC units may be considered advanced theological studies (beyond baccalaureate) highly desired Ability to travel extensively within AHHS service area in all types of weather. Flexibility in scheduling to include evening and weekend presentations and willingness to participate in on-call is required. The chaplain is required to make visits to private residences which may not be accessible to handicapped. May be exposed to and/or life threatening communicable diseases. Must be able to walk up to 5 flights of stairs. Ability to lift/carry up to 50 pounds including presentation materials such as overhead projector, handouts and patient related materials including standard cleaning supplies for personal use, barrier protection for bag and any sacraments to be administered. Must carry a pager.If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.Purpose:The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required.The hospice chaplain provides spiritual care- support and counsel to patients and their families/significant others. He/she serves as a liaison with local communities of faith.Accountabilities:Responsible for the assessment of spiritual and religious needs of patients and families and for arranging for the provision of spiritual and religious support in accordance with the wishes of the patient and family.Responsible for achievement of agency- department and personal goals and objectives.Responsible for the development and enhancement of the spiritual and counseling support services provided by hospice in collaboration with other health care providers- community clergy and other members of the hospice interdisciplinary team.Participates in the QA/CQI process for Advocate Hospice.
