Come join our journey as we create THE destination for top talent in the Healthcare community and set a new standard for how care kidney should be done.Strive Care Partners Strive Care Partners (SCP) is a dedicated business unit within Strive Health that partners with nephrologists to share and succeed in global risk contracts with payors. SCP will benefit patients with CKD and ESKD through a comprehensive, patient-centered approach that enhances lives and local communities. SCP develops local market partnerships with nephrologists and provides them with the tools necessary to be successful in managing risk on their patients. Specifically, SCP delivers primary care services, specialized clinical programs, data integration and analytics, care teams, and administrative support services - all embedded within the nephrologist's practice. SCP contracts with local market payors on the nephrologists' behalf to take global risk on their CKD and ESKD patients and provides nephrologists with meaningful short-term and long-term financial incentives, including participation in shared savings and equity-based programs. Medical AssistantUnder direct supervision of the provider and nursing leadership, performs a variety of administrative and clinical tasks to assist in the quality and compassionate kidney care provided to our patients. Essential FunctionsGreeting and escorting of patients to the exam roomMeasurement and recording of vital signsPreparing patients for the examination/procedure Assisting the provider during the examination/procedureFront desk duties including, but not limited to answering telephone calls, retrieving medical records from doctor's offices; recording charts in the medical recordInputs patient's medication list in the EMR and updates accordingly Update EMR to correctly reflect patient's pharmacy preferenceRecords patient's medical history, allergies, family history, social history, reason for visit, and information such as test results, patient complaints, prescription refills and physician orders in the medical recordAppointment scheduling - intital appointment and reschedule no show patients Patient intake and check outInsurance verificationPatient appointment remindersComply with quality assurance practicesCollect and process specimensPerform CLIA waived/non-waived testsAdminister vaccines as allowe/dictated by state requlationClean(sanitize) and restock exam rooms after each patient exam and at the beginning and end of each work dayKeep logs of all tests orderedRun lab controls dailyPrint lab ordersDistribute lab results, reports, etc. to appropriate physiciansMaintains confidentiality of patient information and records at all timesCompletes patients registration including verifying consents, demographic informationPharmacy and prescription follow upsDressing changesPerdorm specialized testing such as EKG's, vision screening and hearing testsEnter data from monitoring and tracking systems Dispose of medical waste correctlyMeet in person with internal and/or external stakeholders to facilitate team and business priorities/opportunities. Serve patients in person and in multiple care settings (e.g., patient home, clinics) in addition to virtual visits. QualificationsMinimum QualificationsBLS Certification Graduate from a Commission on Accreditation of Allied Health Education Programs (CAAHEP) or Accrediting Bureau of Health Education Schools (ABHES) Medical Assistant Program, OR Graduate of a formal medical assistant program, OR Graduate of a Licensed Practical Nurse (LPN) program. 2 years clinical experience, ie; hospital, doctor's office, clinic, etc...2 years experience in lab collection, EKGs, and other office tests Preferred QualificationsAbility to perform a wide variety of clinical skills as related to ambulatory patient careGood communication and customer service skills Detail oriented and highly organized individual Ability to prioritize, multi-task, and problem solve Basic computer knowledge and keyboarding skills. Customer service experienceIntermediate proficiency in MS Word, Excel, PowerPoint, and OutlookExcellent verbal and written communication skillsSkilled at dealing with confidential information and/or issues using discretion and judgmentCommunicates clearly, respectfully, and thoughtfully Strive Health offers competitive compensation and benefits.
