work in Chicago, IL, or New York, NY)Role DescriptionThe Oak Street
Health (OSH) Engagement, Experience, and Service Ops team is
responsible for Maximizing the number of patients, both new and
existing, that are engaged in our care model through the delivery
of an unmatched patient experience; Ensuring that we capture
value-based economics on all patients we serve; and Making work
easier for center teams by centralizing standardized activities.The
Patient Engagement & Retention team at Oak Street Health plays a
critical role in maximizing the number of people that Oak Street
Health serves and ensuring that they remain engaged with Oak
Street's care model from the moment they complete their first visit
with Oak Street. The Engagement team is focused on driving
strategic initiatives to increase and retain patient membership
panels, forecasting metrics related to patient visit completion,
and addressing root causes of patient attrition.The Manager,
Patient Engagement will report to the Associate Vice President,
Patient Engagement. This role is focused on using analytics and
data analysis, executive communication, and cross-functional
collaboration to improve patient retention at Oak Street Health.
This individual will partner closely with leadership across the
organization, in particular the Patient Relations Manager (PRM)
program, the Patient Experience team, and field leadership. This
role will support all ongoing efforts to increase patient
retention. Specific efforts will include strategic planning,
operationalizing initiatives, leading ongoing data and reporting
processes, and synthesizing key findings from qualitative and
quantitative analyses.The Manager will be responsible for
independently owning and pushing forward initiatives based on
multiple years of experience with value-based care economics and
managed care growth. Further, the Manager will oversee the direct
supervision of 2 early-in-career professionals.Core
Responsibilities
- Lead all central analytical support functions for the 300+
Patient Relations Manager (PRM) cohort, in partnership with PRM
strategic leaders.
- Manage comprehensive reviews of patients who have attrited from
Oak Street Health, including surfacing the key drivers of attrition
and implementing recovery efforts.
- Drive analytics and lead ongoing reporting to inform strategy
development, enhance partnerships, and develop recommendations to
improve patient retention initiatives.
- Lead the forecasting of annual wellness visit targets for
existing Oak Street Health patients.
- Collaborate with Oak Street Health team members and
stakeholders across Patient Engagement & Population Health to align
retention initiatives with organizational goals and
priorities.
- Partner on financial reconciliation processes relevant to
patient engagement and retention.
- Develop materials and lead discussions with internal
organizational leaders.
- Advise leadership on patient engagement and retention
strategies.
- Manage, coach, and develop two team members.
- Other duties as assigned.What are we looking for?
- 5+ years of experience at a top tier consulting firm,
investment bank, private equity firm, or other comparable
experience.

detail and experience leveraging data / analytics to solve
problems.

- Interest, experience, and/or passion for healthcare and/or
value-based care.
- Proficiency in Excel/Google Sheets, Powerpoint/Google Slides,
Alteryx, and SQL is required.
- Previous strategic planning and operations experience is a
plus.
- Experience with CRM systems (e.g., Salesforce) and other
sales/patient engagement tools is preferred.
- Effective and flexible team player with demonstrated success in
a fast-paced environment.

RangeThe typical pay range for this role is:$66,330.00 -
$145,860.00This pay range represents the base hourly rate or base
annual full-time salary for all positions in the job grade within
which this position falls. The actual base salary offer will depend
on a variety of factors including experience, education, geography
and other relevant factors. This position is eligible for a CVS
Health bonus, commission or short-term incentive program in
addition to the base pay range listed above.
