Combined, Advocate and Aurora are recognized for clinical excellence.Accountabilities:Oversees all aspects of referral management to hospice services, including work que oversight, insurance requirements, and customer service standards. Ensures workflow processes are timely, in compliance with regulatory requirements, and cost efficient. Responsible for coordinating the service needs of the referral including the scheduling of the admission. Enforces high patient focused customer care standards through regular tracking and monitoring of phone interactions. Acts as a conduit for entry into hospice care the same day as referral where ever possible. Responsible for meeting established service quality goals, including timely scheduling of referrals. Identifies trends in audits and communicates results to the Director. Performs human resources responsibilities for staff which include interviewing and selection of new employees, promotions, staff development, performance evaluations, compensation changes, resolution of employee concerns, corrective actions, terminations, and overall employee morale. Rounds on team members regularly. Recommends operating budgets and controls expenditures within approved budget objectives. Executes defined growth plan initiatives. Works with business development for management of community based referrals. Identifies business opportunities and community needs, and partakes in developing and implementing strategies to take advantage of opportunities.Position Requirements:Bachelor's Degree in nursing, other health related field or business development. Masters preferred in applicable field.Typically requires 3 years of experience in hospice/home care or call center.Prefer 3 years of supervisory experience in management of staff or lead role.Must have thorough understanding of home health or hospice regulations, reimbursement and care requirements.Strong communication skills.Demonstrates an ability to organize, plan and delegate work effectively an efficiently.Demonstrates an ability to resolve conflict, motivate staff and interact with superiors, peers and subordinates in a direct and professional manner.Demonstrates an ability to evaluate situations according to established standards and policies. Ability to solve problems and make independent judgements and decisions based on experience and good judgement.
